PCBA Component After-Sales Claim Form

Last updated on Jan 26, 2026

We regret to learn that for Order No. [Please enter order number], an anomaly has been identified involving component [Reference Designator + Component Code].

To help us resolve the issue efficiently, please provide the following information as completely and accurately as possible.

Section 1: Description of the Issue

  • Order Quantity:

  • Defective Quantity:

  • Defect Rate:

  • Photos / Videos of the Defective PCBA and Affected Component(s):

Please clearly mark the reference designator(s) of the defective component(s) and clearly demonstrate the failure symptoms (e.g., cracking, soldering defects, no oscillation, short circuits, incorrect electrical values, etc.).

  • Defect Distribution Pattern:

☐ Random distribution

☐ Board edge

☐ Specific module / area

☐ Entire board

Section 2: Component Information and Design Documentation Requested

1. Component Datasheet Verification

Please review the component datasheet and test whether the measured resistance/value of the component matches the datasheet specifications.

If discrepancies are found, please provide:

  • Screenshot of the specified value from the datasheet
  • Photo of the measured value
  • Video showing the abnormal measurement process

2. Design and Application Review Materials

  • Relevant schematic sections:

Please provide the schematic page(s) showing the affected component and its surrounding circuitry.

  • PCB fabrication and DFM review:

Please check whether there are any issues in the Gerber files or DFM drawings. If issues are identified, provide annotated images highlighting the error locations.

  • Critical operating parameters verification:

Please confirm that the voltage, current, frequency, and power applied to the component in the design are strictly within the limits specified in the datasheet.

3. Samples and Test Support

  • Good vs. defective sample comparison:

Please perform comparative testing between good samples and defective samples, and provide test data comparisons along with supporting photos or videos.

  • Customer-side test results:

Was the issue reproducible during customer-side testing?

If so, please describe the observed behavior and test conditions.

4. History and Change Information

  • Is this component used for the first time in this order, or is it an alternate / substitute part? Have you previously placed orders with the same design at JLCPCB?

If yes, please provide the previous order number(s).

  • Compared with previous successful orders, have there been any changes in:

BOM

Component part numbers or markings

Component suppliers

PCB design

If yes, please provide detailed descriptions of the changes.
